‘Minimally-Invasive Gynecologic Surgery: MRI-Guided Focused Ultrasound for Adenomyosis, and Other Engineering Challenges’

Abstract:

Adenomyosis is a common gynecological disorder that usually affects premenopausal women over 30 years of age, and is characterized by the infiltration of endometrial glands and stroma into the muscular body of the uterus. Hysterectomy is the definitive treatment for adenomyosis, but the disorder is increasingly diagnosed in women who wish to preserve their fertility. For those women there is currently no effective treatment option. The success of magnetic resonance imaging-guided focused ultrasound (MRgFUS) in treating uterine fibroids makes it an attractive potential treatment for adenomyosis. However, because adenomyotic glands are smaller and more widely distributed throughout the myometrium, current MRgFUS systems lack the precision and efficiency to treat the disorder in a clinically-feasible time while ensuring that adjacent myometrial and overlying endometrial tissues are not damaged.

We will present our current progress in developing new real-time MR imaging and FUS control approaches to enable safe and complete MRgFUS treatment of adenomyosis. With these developments, MRgFUS could become the first fertility-preserving treatment option of this disorder
