Minor Declaration

Two easy steps:

1. Download and fill out the Undergraduate Declaration of Major/Minor Form.

2. Email completed form to chad.b.smith@vanderbilt.edu

You will be assigned an adviser and the adviser signature will be provided. You will then be instructed to turn the signed form into your school’s Academic Services office for processing.
