Departure

If you are completing your program at Vanderbilt and leaving the U.S. please complete the online Departure Form. After your program ends your DS-2019 will be inactivated and you will not be able to use the DS-2019 for reentry to the U.S.

J-1 Student Intern Evaluations

All programs must conduct a concluding evaluation prior to the conclusion of the internship, and both the student intern and immediate supervisor must sign the evaluation forms. The evaluation form can be found here.

Programs lasting more than six months must conduct both a midpoint and concluding evaluation. Submit a copy of all evaluations to ISSS. ISSS must retain these evaluations for three years following completion of the student’s internship.
