The Office of Student Financial Aid and Scholarships utilizes NextGen Dynamic Forms for our online digital forms.

Participants in Form Completion

Additional Help: 

There are two primary roles involved in form submission: Initiators and Cosigners.

  • Initiators: Typically, this will be the student or a representative from the financial aid office.
  • Cosigners: Parents or the studen's spouse often fill this role. All participants must complete their respective sections for a form to be processed.

How to Complete the Form

Users will have access to type or click only on the portions of the form that require their particular input. All other sections will appear faded to indicate they are for other contributors.

Email Notifications for Cosigners

Cosigners will receive email notifications from notify@ngwebsolutions.com. Please add this email address to your safe senders list to ensure these communications are received. Doing so will prevent important notifications from filtering into junk or spam folders.

We cannot accept documents via email that contain personal identifying information including, but not limited to social security numbers, addresses, & date of birth. Undergraduate First Year Applicants, please upload documents using your Financial Aid Checklist in MyAppVU.  Undergraduate Returning Students, please submit all documents via your YES Financial Aid Checklist.

Once you have successfully completed the form, please allow up to 24 hours for your checklist to be updated accordingly.

  • Q: My parents have not received an email containing the link to the form required for their part - what should be my course of action?
    • A: Advise them to inspect their spam or junk folders for an email from notify@ngwebsolutions.com .
    • A: If they still do not get the email, you'll need to fill out the form again using a different email address.
  • Q: How long will it take for my form to be processed and my checklist to be updated?
    • A: It usually takes about 24 hours for your checklist to show as complete after submitting the form. Keep in mind that weekends and holidays might extend this timeline.
  • Q: What can I do if I need to edit my form after signing it and sending it to cosigners?
    • A: Once a form is signed and sent, you cannot edit it. You will need to fill out and submit a new form if changes are required.
  • Q: How can I update the email address I provided for my cosigner?
    • A: If you have entered the wrong email address for your cosigner, you must submit a new form with the corrected information.
